Disclosure of Ownership Notice

I have been informed prior to my surgery/procedure that the physicians who perform surgery/procedures at Center for Advanced Surgery may have an ownership interest in the Center for Advanced Surgery, Dr. David T. Vroman, Dr. Millin Chandu Budev, Dr. D. Virgil Alfaro III, Dr. Sabino J. D’Agostino, Dr. Kristiana D. Neff, and Dr. George O. Waring IV. I have been provided a list of physicians who have a financial interest or ownership in the Center. The physician has given me the option to be treated at another facility/Center, of which I have declined. I wish to have my procedure/surgery performed at Center for Advanced Surgery.
